Meet Stacy
Stacey Belyeu-Guinn FNP-C is the lead provider for Arlington Occupational Health and Wellness. She was trained at The University of Texas Medical Branch and Texas Women’s University.
Stacey brings over 15 years of experience in emergency medicine, family medicine and bariatrics. From a very young age her personal goal has always been about advancing people’s lives by helping with their general health and wellness.
Stacey enjoys spending time with her husband and daughters when she has free time from changing lives.

Cancellation Policy
Your appointment time is reserved exclusively for you. To avoid charges, please notify us at least 24 hours in advance if you need to cancel or reschedule. If we do not receive notice within 24 hours, a charge of $25 will be applied to your account. Late cancellations will be applicable to $25 fee if arrival after 30 minutes of appointment. “No shows” will be charged 100% of the fee.
HIPAA Patient Rights
The Health Insurance Portability and Accountability act of 1996 (HIPAA) protects the privacy and security of an individual’s identifiable health information and gives individuals the ability to access and obtain and copy of their health information. With limited exceptions, the HIPAA Privacy Rule (the Privacy Rule) provides individuals with a legal, enforceable right to see and receive copies upon request of the information in their medical and other health records maintained by their health care providers and health plans.
Refund/Exchange Policy
While we make every effort to make our customers as happy as possible, all service sales are final. We are a small business and unfortunately cannot offer refunds on any used or unused appointments for individual services, service packages or products.
